Dementia Care
At Holme Manor we understand that dementia affects people in different ways, meaning every resident’s needs are unique. Our building allows residents freedom to move around the home but also be in a friendly, stimulating and safe environment. Our specialist dementia care is designed specifically to enhance the quality of life of our residents. Our staff provide exceptional care along with unique dementia care knowledge, meaning that every individual will receive the personalised level of care and support they require. Also as their needs change plans will change accordingly along with support.

Also we provide access to a wide range of social and creative activities, to encourage good mental health, well being and social opportunities. Our Activity Co-ordinator will work very closely to find out valuable information from the resident but also family and friend to ensure that they can continue hobbies and interests throughout their stay at Holme Manor.

Relationship – Centred Care

We recognise that the transition into a care setting can be an emotional time for both residents and their family. Our relationship with every resident and their family and friends is integral to ensuring a smooth transition, enabling residents to continue important routines, hobbies and interests all ensuring that dignity and wellbeing is kept.

Our care approach is both personalised and collaborative, recognising that meeting residents needs and wishes is essential and that this is a partnership approach based on strong and positive relationships. This is described as Relationship-Centred Care and is at the heart of everything we do.
